  4. Pengesahan sebagai Badan Hukum
  5. Pengesahan Akta Perubahan
  6. NPWP Badan Usaha
  7. NIB (Nomor Induk Berusaha)
  8. Identitas (KTP) Penanggung Jawab Badan Usaha
  9. Laporan / Neraca Keuangan Tahun Terakhir / Tahun berjalan yang di ttd diatas materai dan di stample (Dari Akuntan publik lebih baik)
  10. Struktur Organisasi Badan Usaha

*Harus sampai PJT & TT (Komisaris tidak boleh jd PJT/TT)

*Struktur Jika dibawah Direktur Utama ada Direktur, maka Direktur Utama tidak boleh jadi PJT

*Direktur boleh jadi PJT jika struktur diatasnya tidak ada jabatan lain

  2. Izin Prinsip Penanaman Modal (Jika PMA)
  3. Izin Prinsip Penanaman Modal Perubahan (Jika PMA)

  1. Sertifikat Teknis (Sertifikat Kompetensi) PJT & TT untuk setiap bidang dan sub bidang yang dimohon
  2. Surat Penunjukan PJT & TT untuk setiap bidang dan sub bidang yang dimohon yang ditandatangani oleh direksi dan PJT/TT (Form PS.3.3 & PS.3.4) *Jika PJT Direktur, yang ttd “kolom menunjuk” harus Komisaris/Pejabat di atas Direktur
  3. Surat Pernyataan Kesediaan sebagai PJT & TT yang ditandatangani di atas meterai (Form PS.3.5 & PS.3.6)
  4. Surat Permohonan Penggantian PJT di SBU *Jika ada perubahan PJT (Form PS.3.8)
